Shelly, the application needs to be filled out in its entirety.  

I specifically requested that you provide the children's full names / dob on the FSP form, as we need to verify they are the same children we have these grants created for.  We ask this of families using different teams or agencies.  Grant funds will not be disbursed without this information.
(We have a facilitator who is qualified to seek verification on the availability of our children and had already done so at this point.  Also, the US Embassy specifically told us that neither RR or Serge (their facilitator) had the authority to be the ones to verify the children.  Also, we had already had the organizations of Grace Haven and Life2Orphans (both of whom have had direct contact with the children and orphanage) verify that the children we were adopting are indeed the ones with the grants.)

The homestudy agency/ social worker agency information is incomplete.  (It was never incomplete.  Our facilitator only wanted her email address to be given. In Texas your social worker does not have to work for an agency if the adoption is from a Non-Haque country).

The facilitator / adoption agency information is missing -- one or the other should be filled out, as you've selected "Other"(Our facilitator specifically did not want RR to have their contact info.  Also, the Embassy told us that this information is not needed by a grant organization since that organization is not an agency)

We can not approve your application, as it currently is.  Please redo the forms, and resend.

You will see that we did not sign the line giving any permission for RR to talk to anyone regarding our adoption, yet Andrea contacted USCIS .  We also originally had our sw send in an email rather than the preapproval form since the preapproval part did not apply as we already had a completed home study.  However, after that we did have our sw send the Preapproval form in
